please read this information about our school flu vaccines

This flu season we would like to help protect our student body by providing flu vaccines during school hours.  The Carroll County Health Department will be offering and administering flu shots to Central Elementary School students on October 22 from 9-11am. Please be aware that participation is completely voluntary and vaccines will only be administered to those students who have a parent signed consent form. If your child has health insurance the Carroll County health department will bill your insurance provider. If you would like for your child to receive the flu vaccine at school, please review the Vaccine Information Sheet and then print, sign and return the Flu consent form to the school nurse by Monday, October 15.  The Vaccine Information Sheet and the flu consent form are available on our school website. If you do not have access to these forms online there are copies available in the front office or you can request one be sent home by contacting the nurse. If you have questions regarding the flu vaccine or are unsure if your child should receive the flu vaccine please contact your healthcare provider.
